Under Eye Filler for Hollow Eyes: What to Know Before You Treat Tear Troughs

Under-eye filler is one of the most requested treatments in aesthetics right now (and honestly, one of the most misunderstood).


When done correctly, tear trough filler can soften hollowness, reduce that constantly tired look, and help patients appear naturally refreshed without surgery. But when it’s overdone, poorly placed, or injected into the wrong patient? That’s when you start seeing the puffiness, filler bags, and unnatural results that give under-eye filler a bad reputation.

As an injector, I’m extremely conservative with the under-eye area because this is not a treatment that should be rushed, overfilled, or approached with a one-size-fits-all mindset.

What Causes Hollow Under Eyes?

Hollow under eyes can happen for several reasons, including aging, genetics, collagen loss, volume loss through the cheeks, and thinning skin beneath the eyes.


A lot of patients come in thinking they have “dark circles,” when in reality, the issue is often shadowing caused by structural volume loss.


As support decreases through the midface over time, the area beneath the eyes can start to look sunken, tired, or heavy...even when you’re fully rested.

What are Tear Trough Fillers?

Under-eye filler, also called tear trough filler, uses hyaluronic acid (HA) filler to restore subtle support beneath the eyes.


The goal is not to completely erase every line or aggressively fill the area. In fact, that’s usually when problems happen.


A beautiful under-eye result should soften hollowness and create a smoother transition between the cheek and under-eye area without looking obvious or puffy.


When done correctly, people usually notice that you look more rested (not that you had filler).


What Fillers Are Commonly Used for Hollow Eyes?

At Flawless Aesthetics, filler selection is customized based on the patient’s anatomy, skin quality, degree of volume loss, and overall facial balance. In the under-eye area, I typically prefer softer hyaluronic acid fillers with lower hydrophilicity (meaning they attract less water and are less likely to create puffiness in delicate tissue). Fillers from the Restylane family, the RHA Collection, and Belotero are commonly used in this area depending on the patient.


That being said, there is no single “best” filler for hollow eyes. The best filler is ultimately the one that best matches the patient’s anatomy, skin quality, and individual edema risk.

Why Under Eye Filler Sometimes Looks Bad

Let’s be honest... under-eye filler has gotten a bad reputation online lately, and in some cases, deservedly so.


The under-eye area is delicate. When too much filler is placed, poor technique is used, or someone who isn’t a good candidate is treated anyway, the result can actually make the eyes look heavier, puffier, or more swollen over time.


Common issues I see include:

  • Puffiness

  • Swelling or water retention

  • Filler migration

  • Uneven texture

  • A heavy or “baggy” appearance


I often see patients who think they need more filler when the real issue is poor skin quality, weak midface support, or filler that was improperly placed to begin with. This is exactly why under-eye filler should never be treated like a quick “lunchtime filler” procedure, but a process.


Not Everyone Is a Good Candidate for Under Eye Filler

One of the most important parts of tear trough filler is understanding who should not be treated.

Patients with significant under-eye bags, chronic puffiness, poor lymphatic drainage, severe skin laxity, or very thin crepey skin are often poor candidates for filler alone. In some cases, filler can actually make these concerns more noticeable rather than improve them.


I also always remind patients that not every under-eye concern is caused by volume loss. Sometimes the issue is related to pigmentation, skin translucency, cheek descent, or orbital fat prolapse rather than true hollowness beneath the eyes.

Why Skin Quality Matters Before Treating the Under Eyes

One of the biggest mistakes I see in aesthetics is trying to “fix” poor skin quality by simply adding more filler.


If the skin beneath the eyes is thin, crepey, or lax, filler alone may not create a beautiful result. In some cases, it can actually make texture more noticeable.


That’s why I often recommend improving skin integrity first with treatments like Sylfirm RF microneedling before placing under-eye filler.


RF microneedling helps stimulate collagen, strengthen the skin, and improve texture around the eyes. For some patients, one to three sessions can dramatically improve the quality of the under-eye area before filler is ever placed.


Creating a healthier foundation first almost always leads to a better long-term result.

Rather than overfilling the area all at once, treatment is often performed gradually in a series when appropriate. We also assess the full face- including cheek support, skin quality, and facial balance- before recommending treatment.


In many cases, restoring support through the midface first can improve the under-eye area more naturally than placing filler directly beneath the eyes alone.


My goal is never to make patients look “done.” I want patients to still look like themselves- just brighter, smoother, and more refreshed.


Are Under Eye Fillers Worth It?

For the right patient, under-eye filler can make a huge difference in helping the face appear more rested and refreshed without looking obvious or overdone.


Results vary depending on the patient, the filler used, and metabolism, but under-eye filler can often last anywhere from 9 to 18 months. Because the under-eye area moves less than areas like the lips, filler may last longer here than many patients expect.


That being said, I always prefer a conservative approach to maintenance rather than repeatedly adding large amounts of filler over time. In many cases, the best long-term results come from combining treatments that support both facial structure and skin quality rather than relying on filler alone.


The key is proper assessment, conservative technique, and understanding when filler is (and isn’t) the right solution.
